Keith Wille
Keith Wille, MD, MSPH is a Professor of Medicine at the University of Alabama at Birmingham, where he has been a longstanding member of the Pulmonary and Critical Care faculty since completing his fellowship training there. Dr. Wille’s clinical and academic work centers on critical care medicine, with particular expertise in mechanical circulatory support technologies, including ECMO and other advanced life support modalities. He has a strong interest in transplantation and the management of complex cardiorespiratory failure, particularly in patients requiring multidisciplinary, high-acuity care. In addition to his clinical work, Dr. Wille has pursued advanced training in research and quality improvement, earning a Master of Science in Public Health in Clinical Epidemiology from the UAB School of Public Health in 2013 and a Master of Science in Healthcare Quality and Safety (MSHQS) in 2021. He currently serves as Medical Director of the UAB Adult ECMO and Advanced Lung Diseases Programs and plays a key leadership role in advancing mechanical circulatory support therapies. He is actively involved in national and international committee work focused on mechanical circulatory support and critical care, including the Extracorporeal Life Support Organization, American College of Chest Physicians, and American Thoracic Society, contributing to guideline development, education, and collaborative initiatives that shape practice in these fields. He has contributed to several research consortiums, including the Lung Transplant Outcomes Group (LTOG) and the International Severe Acute Respiratory and Emerging Infection Consortium, supporting collaborative research in critical illness and advanced cardiorespiratory disease. He has authored or co-authored more than 100 peer-reviewed articles in critical care, mechanical circulatory support, and transplantation, reflecting his commitment to advancing both clinical practice and research.

  • Ashita Tolwani Speaker The ABC of the CRRT prescriptionPrescribing continuous renal replacement therapy requires an understanding of how modality, blood flow, treatment dose, replacement-fluid delivery, and circuit factors influence therapy delivery. This presentation will provide a practical framework for developing and evaluating a CRRT prescription, including the principles of diffusive and convective clearance, prescribed versus delivered dose, and the impact of replacement-fluid location and treatment interruptions. Clinical examples will highlight common prescribing challenges and demonstrate how CRRT settings can be adjusted to meet changing patient needs while maintaining effective and safe therapy.AnticoagulationEffective anticoagulation is essential to maintain circuit patency, minimize blood loss, and ensure delivery of the prescribed CRRT dose. This presentation will review the principles guiding anticoagulant selection during CRRT, including the relative roles of regional citrate anticoagulation, unfractionated heparin, and alternative agents. Particular emphasis will be placed on balancing circuit longevity with patient safety, recognizing patients at increased risk for bleeding or citrate intolerance, and monitoring for metabolic and anticoagulation-related complications. Dysnatremia and acid-base disordersContinuous renal replacement therapy can be adapted to safely manage severe sodium and acid–base disturbances. This presentation will review a practical approach to modifying the CRRT prescription based on the patient’s serum sodium, acid–base status, and desired rate of correction. Emphasis will be placed on anticipating how CRRT solutions and treatment settings influence biochemical changes and on avoiding overly rapid correction or treatment-related complications. Clinical examples will illustrate key principles for individualized therapy.Severe hypernatremia and hyperkalemia in AKI requiring KRTManaging Patients With Combined Kidney and Liver Failure
